Children Needed for Kidney Function Study

Thank you for your interest in the CHORI Kidney Function Study.



Will My Child Be Paid for Being in this Study?:
  • If you and your child return the signed consent and assent forms and your child gives a urine sample at a Quest Diagnostic clinic, you will receive a $40 Amazon.com® Gift Card to shop at www.amazon.com* with your child.
  • If your child also returns a completed questionnaire, your child will get an extra $10 Amazon.com® Gift Card.
  • CHORI will email your gift cards within 6 weeks of CHORI receiving the signed forms and Quest Diagnostics report.

In Order to Participate in this Study, You Must:
  1. Complete THREE Study Forms (Parent Consent form, Child Assent form, and Study Questionnaire)
  2. Make one visit with your child to a local Quest Diagnostic clinic on a weekday morning between 8:30 and 9:30 am for your child to give a urine sample
  3. Return completed forms to CHORI
